Medical Release Form
A signed copy of the North Carolina DeMolay Medical Release and Consent form must be on file with your chapter records for all members under the age of 18. This form must be signed by the parent and/or legal guardian and notarized by a Notary Public.
Photo Release Form
A signed copy of the North Carolina DeMolay Photo Release form must be on file with your chapter records for all members, regardless of age. If a member is under the age of 18, this form must be signed by the parent and/or legal guardian.
Cardinal Court of Chevaliers Application
The Cardinal Court of Chevaliers is open to any Active DeMolay or Senior DeMolay who has received the Degree of Chevalier. Members of the Court meet annually and assist North Carolina DeMolay through various activities during the year.

Form 10
A Form 10 should be completed for each new candidate initiated into DeMolay.
Adult Worker Form
This form should be completed by all new adult volunteers.

North Carolina Rainbow for Girls
The International Order of Rainbow for Girls is the sister organization of DeMolay. Rainbow is a non-profit, character building service organization. Rainbow is open to girls between the ages of 11 and 20. You can be assured that the values promoted by Rainbow are strong and basic to many beliefs.
